On 19 July 2024 a faulty content update for a widely used security product crashed about 8.5 million Windows computers. Using public information (the vendor's published root-cause analysis and news reports) and this chapter, write a failure autopsy of about one page:
- What happened — 3–4 sentences, facts only.
- Chain of causes — at least four links, from the first mistake to the global outage (think: code, validation, testing, deployment, operation).
- Lifecycle mapping — for each link, the lifecycle activity where it could have been stopped.
- Prevention — three concrete engineering practices, each linked to a specific cause.
